Register to Have Your Store Participate in a Mystery Shopper Program

In today's world, what you don't know CAN absolutely hurt you. For your business, it's important to know information about what really happens at the point of contact between the company and the customer. 

MidStates HPBA is excited to announce the 2012 Mystery Shopper program to help you and your business be successful. As a member of MidStates HPBA, you have the opportunity to register for the 2012 HPBA Mystery Shop Program. 
  
Get registered today to have your store be 'mystery shopped' during the month of November 2012. Registration deadline is October 19, 2012.

Registration is easy!
  1. Complete the Registration Form
    • Registration cost is $55.00 per visit for a standard shop.
    • Registration Form can be emailed to registration@dsgai.com
    • Or mailed with payment to the following address:

  1. Click on the following link for the Location List spreadsheet and complete store location information.
  2. Pay either by check or credit card.
  3. DSG Associates will contact you directly to set up your Mystery Shopper. 
  4. Please have all registration forms mailed by October 19, 2012.
